Medical Release and Authorization

As an Athlete (if the age of the athlete is over 16), Parent and/or Guardian (if the age of the athlete is under 16) of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the athlete, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the athlete’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me or my assigned emergency contacts in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me or my assigned emergency contacts.

Permission is also granted to the Gozo Cycling Club Time Trial (TT) Race Organizers, and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the athlete's admission to the medical facility.

Release authorized on the dates and/or duration of the registered event.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, in my absence.

PERMISSION FOR SOCIAL MEDIA USE

I give the Gozo Cycling Club TT Race Organizers consent to take and use photos/videos of the athlete on the clubs' online platforms and media

Failure to follow rules may result in automatic disqualification and instant banning from the premises.

WAIVER FOR RACING

I declare that I am insured against 3rd party injury. I also understand there are risks inherent with bike riding on public streets and races where many hazards exist. I also understand that there may be a large number of cyclists, some of whom are inexperienced, creating further hazards.

I am voluntarily participating in these events with knowledge of the hazards involved and accept all risks of injury, inconvenience, harm, loss or death. I, or my dependent, am physically capable of participating in events and the equipment I /my dependent will use. Any equipment will be in proper working condition. I acknowledge that I, and I alone, am solely responsible for my/my dependent’s personal health and safety, and the personal property I bring with me. I will read any event description and the rules of participation in the event and I will abide by all rules and regulations established by event organizers and personnel. I agree to wear a helmet, adhere to all other event rules, and conduct myself in a safe and prudent manner while participating in any training or event. I consent to and permit emergency medical treatment in the event of illness or injury, including transportation to a medical facility, and will be responsible for any and all related costs. I give the GCC and the Bike Event Sponsors permission to use my (and any minor’s) name and any photograph, voice or likeness of me during the event in any promotional materials or publications. I consent to and authorize in advance such use and waive my rights of privacy in connection therewith.

Confirmation

BY ACKNOWLEDGING AND SUBMITTING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
